It should just automatically work, as far as I know. Certainly new accounts don't get referred to me or any of the mods for activation.

Try again, maybe adding a hyphen to the nick and using a new email address?

Because of the past problems we've had with trolls on the VCI sites, I've taken advantage of a feature on the new vBulletin software that some of our sites are running:

They hold all new accounts for admin's approval before they get confirmed & activated.

However, I do this every morning and afternoon so there's no reason why she shouldn't have received a confirmation by the end of the day at least...

Unless she entered the wrong email or mis-typed her details...
